What You’ll Do:

• Evaluate patients and develop individualized speech therapy treatment plans
• Provide in-home speech therapy services
• Help patients improve speech, language, communication, cognitive, and swallowing abilities
• Educate patients and caregivers on exercises, communication strategies, and safety techniques
• Monitor patient progress and adjust care plans as needed
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Complete accurate and timely documentation

What We’re Looking For:

• Active Texas Speech-Language Pathologist (SLP) license (required)
• Home health experience preferred
• Strong clinical judgment and patient care skills
• Ability to work independently in a home setting
• Excellent communication and documentation skills
• Reliable transportation & valid driver’s license
• Kinnser/WellSky EMR experience (preferred)
