What You’ll Do:

• Evaluate patients and develop individualized treatment plans
• Provide in-home occupational therapy services
• Help patients improve daily living skills (ADLs), strength, coordination, and safety
• Recommend adaptive equipment and home modifications
• Educate patients and caregivers on techniques to improve independence
• Monitor progress and adjust care plans as needed
• Complete accurate and timely documentation

 What We’re Looking For:

• Active Texas Occupational Therapist (OT) license (required)
• Home health experience preferred
• Strong clinical judgment and patient care skills
• Ability to work independently in a home setting
• Excellent communication and documentation skills
• Reliable transportation & valid driver’s license
• Kinnser/WellSky EMR experience (preferred)
